How personalised treatment goals are developed and reviewed

Treatment goals do not come from a standard list. They develop in conversation, based on a person’s needs, circumstances and priorities.

Treatment goals should be clear enough to guide the work and personal enough to matter to the individual. They are not selected from a standard list. They develop through assessment, conversation and an understanding of the person’s needs, circumstances and priorities.

Beginning with the whole person

The concern that led someone to seek help is an important starting point, but goals may also relate to health, relationships, emotional wellbeing, daily functioning and life after treatment. Strengths, responsibilities and available support also matter.

Setting realistic priorities

Trying to address everything at once can make treatment feel unfocused. Clear priorities help direct attention towards what is most important and achievable at that stage. Goals should be specific enough to review while allowing room for new understanding to emerge.

Personalised care is not a fixed promise. It is an ongoing process of assessment, collaboration and review.

Working collaboratively

Where appropriate, goals are developed with the person rather than imposed on them. Professional guidance remains important, particularly where safety or health concerns are involved, but collaboration can strengthen relevance and personal responsibility. Family perspectives may contribute only with appropriate consent and clinical judgement.

Reviewing and adapting

Goals may change as treatment progresses. Regular review allows the person and team to recognise developments, identify barriers and adjust the focus when needed.
